I am an disabled individual who ask for reasonable accommodations at work. My night mare began.
The company doctor has removed, vital disability files from my folder, changed restrictions without permission, had someone in his office forge my written name and sign my social security number on files to get information ( the person who did this initialed it) Also, a blank piece of paper was in my medical chart, that appears that someone was trying to write my husbands name. That purpose is unknown.
Numerous releases that I did sign for I have nothing to hide, were changed to add informaion that the company had no business too.
Please advise me if this is agaist the law.

If the doctor or his people altered medical records without a statement outlining the reasons for its alteration, it could very well be against the law. Another words, it they changed the diagnosis on previous documents without a current justification, it is most likely illegal, fraudulent, or unethical. At the very least the doctor could have problems with state licensing if the complaints could be proven. Now, many doctor's offices send out medical releases to obtain records from other health care providers and they do sometimes "write" in the person's name and social security number on the request form. They make copies of one that you signed because it is not always known how many are needed or who they will be sent to. This could be an "improper" or illegal act if your signature was used/forged on the releases without your permission or knowledge. You most likely will need to obtain legal counsel for this situation as it appears something could be happening to change your benefits or alter future responsibilities on behalf of the company or their insurance carrier. Find a good attorney in your area that specializes in Worker's Compensation, Disability, Social Security, etc., and let them go to work for you!

It was not a medical release form they forged, it was an actual medical document that a patient is to fill out and take to a doctor to be filled out. What do you think?

It will depend on the nature of the medical document, what was changed, how it relates to your case, if there was any intention to make it look like "you" changed it, etc. There are too many issues to offer better advice on your specific issue. If you feel like the doctor's office altered "anything" improperly, contact a local attorney, the State Medical Board, your disability Commission, or Social Security which ever the jurisdiction falls under. You will need to have some form of evidence or proof the change was made from an original or direct copy of an original through your testimony, and so forth. Look at there possible INTENT and if you think that anything was done to harm your case or affect your future health or treatment possibilities, then find someone and fight it!
