Note Hubbard's use of the term "White Form", or "WF" in the Goodrich Suit Evaluation.

The WF is a case assessment form used on all new cases and at the beginning of Dianetics auditing. It has also been called a PC [Preclear] Assessment Form (ref. HCOB 21 August 1971 C/S Series 1 AUDITOR'S RIGHTS © 1991 L. Ron Hubbard Library).

Hubbard instructed that the case information Goodrich divulged to his auditor be used against him to win the subject law suit. Hubbard's guilty knowledge of what he was doing is demonstrated by his use of the term "priest-penitent privilege" (See http://www.gerryarmstrong.org/50grand/cult/goodrich-eval.html#6348 ).

The current rendition of this form is called the Original Assessment Sheet, and is given as an HCOB by the same title, HCOB 24 June 1978RA, New Era Dianetics Series 5RA © 1991 L. Ron Hubbard Library.

The HCOB form is 15 pages long; it is filled out at the start of his/her auditing and also as the first action in New Era Dianetics.

Answers to these questions provide a very complete picture of the person, including, his/her personal habits, names of all family members and relatives (dead or alive), family background including any history of insanity in family members, details about marital partner and any relationship difficulties, educational background, a complete drug history, job history, names of current and past close friends and allies, medical history, psychological problems such as compulsions, repressions, fears, any psych history and any criminal history.

According to the same HCOB,

HOW THE ORIGINAL ASSESSMENT SHEET IS DONE The assessment is done with the preclear on the meter.

The preclear is given the R-factor [reality factor: the auditor's explanation to the preclear about what is going on] that you will simply be asking him for essential data about himself for the purpose given above.

WHERE DOES THE ORIGINAL ASSESSMENT SHEET GO WHEN COMPLETED When completed, the Original Assessment Sheet is kept in the preclear's folder. A note is made in the Folder Summary of the pc's folder that the Original Assessment Sheet has been done.

The preclear answers these questions while in session with an auditor, and is led to believe that his/her answers are confidential, covered under the priest-penitent privilege and will not be used against him/her. At no time is a pc told the truth, that the personal information s/he is divulging in this assessment, and all his/her other auditing, will be used for blackmail, extortion and any and all fair game that the "religious leader(s)" running Scientology deem the greatest good for themselves.
