Some kind soul at the San Francisco Org (83 McAllister Street) sent me a few of their documents to add to my "Scientology's Abusive Release Forms" page.

Here is a resolution adopted by the board of directors of the Church of Scientology of San Francisco:

And here is the "Religious Services Enrollment Application, Agreement, and General Release" they adopted, which basically says: "whatever happens to you is your own fault, so don't even think about suing us!"

I very much appreciate these blank forms, but what I would really like to receive now from people is copies of release forms that they filled out and initialed and signed while they were in Scientology. If you are an ex-Scientologist and you were allowed to keep copies of the forms you signed, you can send them to me at:
