See http://www.junkbusters.com/ht/en/self.html#prohibit for instructions for doing this. That web page says that a Supreme Court decision said that *you* get to decide what qualifies -- if for some reason you decide that supermarket circulars are offensive, you can fill out one of these forms to make them stop sending them.
