Dear Ms. Felske (or Chief Privacy Officer):

I have recent information that Scientology of Toronto (or connected parts of Scientology) has collected extensive personal information about me that you know about.

Under the Federal Personal Information Protection and Electronics Documents Act (PIPEDA) "an organization shall make readily available to individuals specific information about its policies and practices relating to the management of personal information. Thus I am absolutely entitled to a copy of your Code of Privacy. Since it is not on your web site, you may mail or email a copy of it to the above address.

Also under PIPEDA, "Upon request, an individual shall be informed of the existence, use and disclosure of his or her personal information and shall be given access to that information. An individual shall be able to challenge the accuracy and completeness of the information and have it amended as appropriate."

Though there are exceptions, I am probably entitled examine and correct errors in any personal information in any form Scientology has about me no matter if the materials are stored in the US or here in Canada. This is a formal request to do so.

You have a limited amount of time to respond to this letter.

You may deny my request if you cite reasons. You must in that case inform the Office of Privacy Commissioner of Canada. Please cite File 6100-01660.
