NARRATIVE:
On 02/14/96, a meeting was held at the Pinellas County Medical
Examiner's Office, in attendance were: Captain Paul Maser; Sgt.
Mark Teunis; Dr. Joan Wood; Dr. Robert Davis; SAO Inv. Steve
Porter; and Det. Ron Sudler. During the meeting, the facts and
circumstances of this case were discussed and it was brought to
our attention that tests performed after autopsy indicated that
Lisa McPherson was severely dehydrated, which, in conjunction
with a pulmonary embolism, may have been a contributing factor
to her death.
~arther discussion revealed that follow-up investigation was
needed with regard to Lisa McPherson's lifestyle during the
weeks prior to her death. Also, further examination of the
medical credentials, training, and background experience of
persons caring for Lisa McPherson, Hospital records, ambulance
reports, and other related documents would be needed to
complete this investigation.
On 03/08/96, 1 prepared subpoena request letters for the
following information: Lisa McPherson's medical records from
Morton Plant Hospital; Lisa McPherson's medical records from
Columbia New Port Richey Hospital; and Dr. David Minkoff's
personnel records and medical credentials from Columbia
New'Port Richey Hospital. The subpoena request letters were
subsequently delivered to Inv. Steve Porter at the State's
Attorney Office.
Also on 03/08/96, 1 went to the Pinellas County Emergency
Medical Services Office and.met with Mike Albright, 582-2000,
to ascertain more information regarding Lisa McPherson's
transport by ambulance to Morton Plant Hospital on 11/18/95,
following her motor vehicle accident. Mr. Albright formed me
that Paramedic Bonnie Portolano-and EMT Mark Fabyanich were
involved with the transport of McPherson. Fabyanich is no
longer employed by LifeFleet and his whereabouts are unknown.

Bonnie Portolano is currently assigned to the afternoon shift.
I subsequently scheduled an interview with Portolano for
03/14/96, at 1545 hrs. During our meeting, Mr. Albright
informed me that it was not necessary to obtain a subpoena for
the EMS report regarding Lisa McPherson, and he provided me
with a copy of that report, which will be made part of this
case file.
On 03/12/96, at 1430 hrs, I called Brian Anderson, of the
Church of Scientology Office of Special Affairs to schedule
follow-up interviews with Laura Arrunada, Janis-Johnson, and
Paul Greenwood. Also, efforts will be made to ascertain the
identity of, and conduct an interview with, Lisa McPherson Is
caseworker at the church of-Scientology. Mr. Anderson informed
me he would follow through with my requests and will contact me
as soon as the interviews can be scheduled.
Investigation continues, case status remains active.

NARRATIVE:
On 03/13/96, at 1100 hrs, I received a telephone call from
Brian Anderson, of the Church of Scientology Office of Special
Affairs. Anderson informed me that Laura Arrunada was in Mexico
visiting relatives and would not be available for an interview
at this time. Anderson said he would attempt to contact
Arrunada and ascertain when she would be returning to the
United States. Furthermore, Anderson said the dates of 03/13/96
through ((") 15/96 had major significance to the Church of
Scientology and many Activities were planned; therefore, it
would not be possible to interview Janis Johnson or Paul
Greenwood this week. Anderson said he would coordinate the
interviews with Janis Johnson and Paul Greenwood through the
office of Attorney Robert Johnson, and would make an attempt to
schedule the interviews in the near future.
For future reference in this investigation, I called the Morton
Plant Hospital Physician Referral Service, 462-7300, to obtain
a list of infectious disease specialists practicing at Morton
Plant Hospital. Jill, the operator on duty, informed me that
Morton Plant Hospital had two infectious disease specialists:
Dr. David Reifsnyder, 901 N. Hercules Ave., 446-3515; and Dr.
Ben Schaffer, 2424 Enterprise Rd., 799-6255.
On 03/13/96, at 1320 hrs, Detective Carolyn Carrasquillo and I
went to Bob Lee's Garage, 812 Park St, Clearwater, 4461051, to
interview Joseph MacDonald, W/M, 06/15/56, 11575 Dauphin Ave,
Largo, 34648. MacDonald was the driver of the other vehicle
involved in the motor vehicle accident with Lisa McPherson.
During the interview, MacDonald stated the following:
On.11/18/95, around 550 p.m., he was driving his pick-up truck,
which was pulling a boat, southbound on S. Ft. Harrison Ave. i
in the inside lane, approaching Belleview

Boulevard. A separate accident had occurred near the
intersection, and several emergency vehicles were on the scene.
MacDonald stopped his truck at the intersection, behind
approximately three other vehicles, and was struck from behind
by a vehicle driven by a W/F, approximately 510811, with short,
wavy, brown hair with blonde highlights" later determined to be
Lisa McPherson. MacDonald was not injured by the collision.
Upon exiting his vehicle, he was approached by the other driver
(Lisa McPherson) who was wearing a white "Flag" (Church of
Scientology) t-shirt, and a pair of white sweatpants. McPherson
approached MacDonald, placed her hands on his shoulders and
began to ask in a frantic manner, "where's the people, where's
the people? 11 MacDonald replied, "what are you talking about?"
McPherson then said something like, "I've got to conform",
"I've got to compose", or "I've got to get control". MacDonald
believed McPherson was emotionally distraught because she
initially spoke in a high pitched, exited voice, her as were
wide opened with a fixed gaze, and she appeared to be "shaken
up". MacDonald did not see any physical injuries to McPherson,
nor did he detect any slurred speech, odor of alcohol, or any
other indication she was under the influence of alcohol or
drugs.
Shortly after the accident occurred, a fireman or paramedic
approached McPherson and she told him to stay away. The fireman
directed McPherson to get back into her vehicle, at which time
McPherson stepped into traffic and nearly was struck by a
passing vehicle. McPherson subsequently returned to her
vehicle, without incident. At this point, a fireman assisted
MacDonald with the removal of his outboard motor attached to
his boat, which was damaged in the collison. Once this was
completed, MacDonald moved his vehicle from the roadway, and
McPherson moved her vehicle to Belleview Bv., where she parked
it in front of an ambulance parked along the north curb of the
roadway.
MacDonald recalled that officer Stonelake of the Clearwater
Police Department was on the scene of the accident and asked
McPherson for her driver's license, registration, and insurance
card. McPherson replied that had to return to her vehicle to
obtain the registration and insurance information. MacDonald
waited alongside the roadway talking to a female motorist
involved in the previous motor vehicle accident. After a few

moments of conversation, this female told MacDonald that the
woman from his accident was removing her clothing. MacDonald
looked down Belleview Bv., and observed that McPherson had
removed all of her clothes and was walking along Belleview Bv.
Officer Stonelake, another Officer, and paramedics approached
McPherson and escorted her inside the ambulance. MacDonald was
too far away to hear any conversation between the paramedics,
officers, and McPherson. MacDonald said McPherson was
subsequently transported from the scene and he had no further
contact with her.
During the days following the accident, MacDonald contacted
McPherson's insurance company, State Farm, and was informed
that McPherson's policy had been canceled in 1994. MacDonald
did not want to forward the accident report to Tallahassee
without contacting McPherson because he did not want her
driver's license to be suspended. MacDonald called McPherson's
residence and A. t a message on her answering machine. He then
called her place of employment, AMC Publishing, and after
speaking with four individuals, he spoke to a female
identifying herself as Benetta, and informed her of the
situation and requested to speak with McPherson. MacDonald was
told that McPherson was not a work, but, Benetta would obtain
the information he needed. A couple of days later, MacDonald
received a call from Benetta, who informed him that McPherson's
insurance company was now Travelers. MacDonald subsequently
called Travelers and informed them of the accident. Travelers
indicated they had not been contacted by McPherson as of yet.
In the following weeks, through conversations with the
insurance company, MacDonald learned that McPherson had died,
and that Jeff Schaffner, McPherson's co-worker, wanted to speak
with him concerning the accident. During a phone conversation,
Jeff Schaffner told MacDonald that McPherson died from a sudden
illness, and her financial matters had not been settled.
Subsequently, MacDonald was contacted by McPherson's mother,
Fannie McPherson, and Ann Carlson, Fannie's sister, when they
arrived in town to handle matters relating to funeral
arrangements and matters concerning Lisa McPherson's estate.
Evidently, they had obtained MacDonald's information from the
accident report, and made arrangements with MacDonald, who
works at Bob Lee's garage, to store Lisa McPherson's vehicle

completed repairs. Lisa McPherson's vehicle was subsequently
transported to, and currently remains at, 812 Park St.
MacDonald said, at the family's request, he removed
miscellaneous papers, pens, and other personal effects from the
vehicle. On this date, MacDonald gave me a small box containing
these items, and requested that I retain them for safekeeping,
until they can be claimed by Lisa McPherson's mother, or other
family representative. The box containing those items removed
from McPherson's car was placed into PCSO/PES, by me, for
safekeeping. MacDonald informed me that he also kept notes on
phone calls and other events relating to the accident and will
provide me with a copy of those notes.

On 3/14/96, at approximately 1540 hours, I conducted an
interview with Sunstar Paramedic Bonnie Portolano, W/F, DOB
10/31/62. The interview was conducted at the Sunstar Ambulance
offices on Ulmerton Road and she gave the following statement:
On 11/18/95, Bonnie and her partner, Mark Fabyanic, were at the
scene of an accident at the intersection of Belleview Boulevard
and S. Ft Harrison Avenue. Upon their arrival, CFD paramedics
were at the scene obtaining
atment refusals from the drivers involved in the accident.
While that incident was being investigated, a second accident
occurred involving a vehicle striking the rear of another
vehicle stopped in traffic. At the request of the FD
paramedics, Bonnie approached the drivers involved in that
accident to ascertain whether or not they had sustained any
injuries and to obtain refusals from them if they did not want
to be treated. She initially spoke to the male driver involved
in the accident and he subsequently signed a Refusal Of
Treatment form. She then approached the female driver (Lisa
McPherson) and found that she did not have any evident injuries
from the accident. She was therefore going to have Lisa sign a
Refusal Of Treatment form. Lisa was asked to move her vehicle
from the roadway, at which point Bonnie realized that Lisa
might have some type of emotional problem because she did not
appear to understand the directions given to her when told to
move her vehicle in front of the Sunstar Ambulance parked on
Belleview Boulevard: However, Lisa subsequently moved her
vehicle to that location. Upon exiting the vehicle, Lisa came
running over to Bonnie and made a dead stop just prior to
reaching her. Bonnie asked Lisa why she was running and Lisa
replied that she was looking for her (Bonnie). She asked Lisa
for hers and Lisa replied that she was having difficulty
remembering her local [~] address because she had just moved to
this area. Lisa then provided her with

an address in Texas. According to Bonnie, it is common in
accident cases for people to be traumatized to a certain degree
and they sometime have difficulty gathering their thoughts
after an accident. However, Bonnie believed that Lisa's
behavior was not the normal behavior usually associated with
this type of investigation. After Bonnie talked to Lisa for a
few minutes, Lisa signed a Refusal Of Treatment form and Bonnie
returned to the ambulance. This ambulance was being driven by
Mark Fabyanic.
As Bonnie and Mark were preparing to depart from the scene,
Mark turned to Bonnie and stated that the female (Lisa)
involved in the second accident was taking off her clothing and
starting to walk toward the ambulance along Belleview
Boulevard. As Lisa passed the ambulance, she was totally nude.
Bonnie exited the ambulance, approached Lisa, and asked her why
she had removed all of her clothing. Lisa told her that she had
taken her clothes off because [~] did not need a body and that
"Nobody knows this but I'm an OT[~]. Bonnie did not know what
Lisa was referring to by this statement. Bonnie then escorted
Lisa to the ambulance unit, laid her down on a stretcher
inside, and covered her with a blanket. While inside the
ambulance, Bonnie asked Lisa what her problem was and why she
was behaving in this manner. According to Bonnie, Lisa had a
unique mannerism and spoke in a very programmed voice. Her eyes
were wide open and she had a fixed gaze whenever she looked at
something or someone. Bonnie asked Lisa why she had removed her
clothing and she replied that she wanted people to think that
she was crazy so they would help her. Lisa also told Bonnie
that she needed help and wanted to rest. Bonnie continually
asked Lisa basic questions but Lisa did not acknowledge the
questions and would just lie on the stretcher with her eyes
closed. The only way Bonnie could get Lisa to respond was to
call her by her name. Lisa would then respond by opening her
eyes, apologizing for not knowing the answer to the question
right now, and asking Bonnie to repeat the question. Bonnie
asked Lisa why she needed help and she replied that she had
been a bad
